WSI podoconiosis foot-care model v0.1 | 18 October 2026

Purpose: illustrate gross acute suffering avoided through additional supported
care for adults with persistent podoconiosis lymphoedema during the first year.
This is not a charity donation-return estimate or complete net-benefit analysis.

Run with Python 3 (no dependencies):
  python3 calculate.py
  python3 calculate.py --budget 100 --scenario central --cost 250

Formula:
  USU = budget / yearly cost * 4.5 attacks * effect retained
        * 3 days per attack * 24 hours * average intensity / 47.25

The 4.5 attack-rate difference comes from GoLBeT's primary randomized trial.
US$200 per person-year is a WSI illustration, NOT a verified current price.
Historical 2,419 ETB / I$314 costs in the companion study are context only:
international dollars adjusted for purchasing power are not US dollars.
Cost, effect transfer, attack duration and USU intensity are explicit assumptions.
All scenario costs are nominal 2026 USD assumptions, not inflation conversions.
Scenarios are not statistical confidence bounds or a ranking of interventions.
The calculator is linear and does not establish unlimited delivery capacity.
No additional adherence multiplier is applied to the trial-based effect.

Only acute attacks prevented are counted. Chronic symptoms, psychological and
caregiver effects, treatment burdens, patient time, household expenses, benefits
beyond year one and deaths prevented are unestimated. No-effect yields zero
gross benefit, not proof of zero net harm. Unknowns use JSON null; the CSV uses
the literal word unestimated, not a numerical zero, for unknown outcomes.
