WSI palliative pain-relief model v0.1 | 18 October 2026

This is an exploratory program-cost illustration, not a donation-return estimate.
Clinical evidence supports cancer-pain relief; the cost and average USU effect
are WSI assumptions, not observed Hospice Africa Uganda program results.

Run with Python 3, no dependencies:
  python3 calculate.py
  python3 calculate.py --scenario central --budget 100 --cost 80

Formula:
  USU = budget / cost per patient-week * 7 days * 24 hours
        * average additional intensity reduction / 47.25
Central: 100 / 40 * 7 * 24 * 1 / 47.25 = 8.8888888889 gross USU.
Cost: nominal 2026 USD planning allowance, not a verified price or conversion.
Patient-week means a budget window, not a stopping rule for actual care.
Funded patient-day equivalents are NOT days made pain-free or unique patients.
The reduction is relative to otherwise available care, averaged across all
funded hours and eligible adults with inadequately controlled cancer pain.
Sleep, interruptions, nonresponse and shortened exposure belong in that average;
hours after death count as zero. No extra success/adherence factor is applied.
No benefit is credited beyond the funded week. Longer care needs further funds.
The reported review response proportion is clinical context, not a multiplier.

Only physical pain relief is counted. Other symptoms, psychological support,
caregiver relief, bereavement, treatment harms, household expenses and deaths
prevented are unestimated. No-effect is zero gross relief, not zero net harm.
No suffering is assigned to death itself. Do not add overlapping pre-death hours.
Unknowns are JSON null or CSV 'unestimated', never numerical zero.
Scenarios are not confidence bounds, a ranking or a claim of unlimited capacity.
No organization has endorsed this USU model.
